We are honored to partner with you in any aspect of the patient services we offer. When you refer a patient to Sleep Health MD, we provide:
- An initial consultation with one of our board-certified sleep specialists
- Sleep studies when necessary, performed in our in-office sleep labs or through home sleep apnea testing
- A follow-up appointment to review results and establish a treatment plan
- CPAP education and equipment provided by our staff
- Intensive follow-up to ensure treatment success – our CPAP specialists handle all CPAP compliance management, even if another company supplies the equipment

- Patient demographics and insurance information
- Reason for referral and suspected sleep disorder
- Relevant medical history, medications, and recent clinical notes
- Any prior sleep studies or PAP compliance reports
