Welcome New Patients
Sleep Health MD welcomes new patients at our Santa Cruz, Freedom, Sunnyvale, and Monterey offices. From your first consultation through testing and treatment, our goal is to find out what is disrupting your sleep and help you feel rested again as quickly as possible. Here is everything you need to prepare for your first visit.
Request an AppointmentWhat to Expect as a New Patient
Your first visit is a consultation with one of our sleep specialists. Your provider will talk with you about your symptoms, sleep schedule, daytime energy, medical history, and medications, and may perform a focused exam of your nose, mouth, and throat. Bed partners are welcome to attend, since they often notice snoring, breathing pauses, or movements you may not be aware of.
Based on your consultation, your provider may recommend an in-lab or home sleep study, lifestyle changes, or a specific treatment plan. Most patients return for a follow-up appointment to review results and begin treatment.
What to Bring to Your First Appointment
- A valid photo ID (driver’s license, passport, or military ID)
- Your insurance card(s)
- A credit card or cash for any copay
- A list of your current medications and supplements
- Copies of past sleep studies and CPAP compliance reports, if you’ve been seen at another sleep center
- Your PAP device or data card if you currently use CPAP, APAP, BiPAP, or ASV therapy
- Any relevant medical records that may help your clinician understand your condition

New Patient FAQs
Do I need a referral to become a new patient?
It depends on your insurance. Many HMO plans require a referral, while many PPO and Medicare patients can schedule directly. Check with your carrier to confirm.
How long is the first appointment?
Plan for a thorough consultation. We recommend arriving a few minutes early to complete check-in, especially if you haven’t filled out your forms in advance.
Will I need a sleep study?
Not every patient does, but many sleep disorders, including sleep apnea, narcolepsy, and periodic limb movement disorder, are confirmed with a sleep study.
Can my bed partner come to my appointment?
Yes. Bed partners often provide valuable details about snoring, breathing pauses, and nighttime movements.
Where can I find new patient forms?
Adult and pediatric new patient packets, in English and Spanish, are available on our patient forms page.
What if I already use a CPAP machine?
Bring your device or its data card, along with any prior sleep study and compliance reports, so your provider can review your therapy.
