Insurance Plans We Accept

Sleep Health MD is contracted with most medical insurance plans, including many managed HMOs and PPOs. We also accept Medicare assignment of benefits for standard and supplemental Medicare. Because we do not maintain an all-inclusive list of every plan we are contracted with, we recommend contacting your insurance carrier to verify that our providers are in network for your specific plan.

How to Verify Your Coverage

  • Call the member services number on the back of your insurance card
  • Ask whether Sleep Health MD and our providers are in network for your plan
  • Ask whether you need a referral from your primary care provider to see a specialist
  • Confirm whether sleep studies require prior authorization
  • Ask about coverage for CPAP equipment and supplies

Coverage for Sleep Testing and Treatment

Because sleep disorders are medical conditions, many plans provide full or partial coverage for consultations, home and in-lab sleep testing, CPAP therapy, and other treatments when medically necessary. Coverage for advanced treatments such as Inspire therapy typically requires documentation, including a recent sleep study and a trial of CPAP. Many plans also require proof of regular CPAP use to continue coverage for equipment, and our CPAP specialists help you stay on track.

Referrals and HMO Plans

Many HMO plans require a referral from your primary care provider before your first visit. Physicians can submit referrals through our referring physicians page.

What to Bring to Your Appointment

Please bring your insurance card(s), a valid photo ID, and a credit card or cash for any copay. Review our new patient information and download your forms from the patient forms page. Once you’re a patient, you can review statements and make secure payments through our patient portal.
